This form is to apply for a scholarship to help out with costs for registration to play in the Elk River Youth Softball Association. If approved you must agree to and complete all fundraising activities and volunteer hours (8 hours plus additional based on percentage).
Once this form has been submitted, the Elk River Youth Softball Association board will evaluate the application and respond back on the decision.
